DEFINITIONS:	Incident -	An unplanned event or chain of events that has, or could have, resulted in an injury, illness, or damage to assets, the environment or company reputation
	Injury / Damage -	An incident with an actual outcome leading to an injury, illness, or damage to assets, the environment or company reputation
	Near Miss -	An incident that under slightly different circumstances (or by chance) could have caused an injury, illness, or damage to assets, the environment or company reputation … but did not.
	Potential Incident -	An unsafe practice or hazardous situation that could result in an incident if not corrected (incident did not occur)
